HOW IT WORKS:

• The first step is completing your new patient forms. Please provide accurate information and follow the instructions carefully.
• After all forms are submitted you will be able to book a phone appointment with a provider.
• Consultation fees are non-refundable once completed. You are not obligated to purchase a treatment plan by submitting these forms.
• If you would prefer not to wait for a phone appointment, do not complete these forms—complete a New Patient E-Consult instead.

*Please Note: If you miss your scheduled phone appointment, your provider may complete your consultation as an E-Consultation. You can book a follow-up phone consult if desired (follow-up fee applies).
